DISCLOSURES

• Automatic billing on the same day every month (credit/debit card required)

• Must cancel 1 week prior to billing date via email to avoid next payment membership@themadaxer.com

• Online bookings recommended to ensure lane availability

• Waiver must be signed before every axe throwing session

• Must show ID upon check-in
